The puncture and maintenance of a peripheral or central vein or an artery are activities that demand multidisciplinary actions. Although nursing has a big interface in the responsibility for inserting the peripheral venous punctures and maintaining and removing other puncture modalities, the information about the punctures needs to be shared with the entire health staff, since part of them is generated by other professionals or can be of assistance in making decisions about the subject. We intend to describe the making and the handling of a tool used to localize the puncture place for the intravenous devices and capable of identifying the kind, caliber and puncture date, that is, indispensable requirements for controlling the puncture place and adjacent areas. The tool, which was created in October-November 2001, was preceded by testing in 72 persons and used in 492 interned clients of both genders. 1480 peripheral and central intravenous places were registered on a daily basis during 62 days of observation. In different situations, the tool admitted for the registration of the dynamic use of the blood vessels and was handled by the multi-professional and muti-disciplinary staff, when the used codes were previously known.
