The present study aims to report the experience of constructing an adequate information system for the specific characteristics of new mental health care modalities, especially for the Day-Care Units. This is a qualitative study that uses life history as an instrument for collecting data, reporting on the construction of this system in 1999, which is used until today. This experience resulted in the elaboration of an instrument that includes quantitative and qualitative information, supplying clinical and social data about each client, as well as clients and family members’ frequency of using the service. On the basis of this study, it may be inferred that this instrument is extremely important, since it gives rise to the possibility for optimizing the information flow and service evaluation. It needs to be highlighted that this construction was realized by a transdisciplinary team, which resulted in a broader instrument.
